Previous incidents serve as warnings that hazard safety shortcomings may have disastrous consequences. Analyzing past incidents provides vital information on these shortcomings, measures for managing the associated risks, and procedures for mitigating subsequent incidents. Finally, understanding the possibility of catastrophic occurrences ensuing from process safety pitfalls helps to establish a sense of vulnerability within an organization, which grows ever more difficult due to the reduced frequency of serious events in the chemical engineering industry in recent years.

I believe the most important piece of advice is to attend safety conferences. Safety conferences provide valuable information on first-hand experience of process safety incidents. Moreover, the conferences serve as forums for sharing and/or learning effectively new process safety developments in the industry and methods of implementation.
